On the following page, we have listed all health insurances that are currently accepted by our organization.
To assist with the registration process on your first healthcare visit, please select, print out, and complete one of the four available health history forms and the corresponding registration form based on the age of the patient.
Here is a list of the health history forms:
- Newborn Health History Form (Birth–3 months)
- Pediatric Health History Form (3 months–11 years)
- Adolescent Health History Form (12 years–18 years)
- Adult Health History Form (19 years-Older)
Please bring all completed forms to your first appointment. This will ensure a smooth registration process.
If you are unable to print the forms, please request a packet when you schedule your appointment, and we will mail it to you. You may schedule an appointment by calling 1-888-296-GPHA (4742).
Please bring the following to all appointments:
- Insurance Card(s)
- Photo Identification
- Co-Pay for the visit, as required by your health insurance
Co-pays and office visit fees are due at the time of service and can be paid by cash, check, or credit/debit cards.
If you are unable to keep your appointment, kindly notify our office 24 hours in advance so that we can reschedule your appointment and re-assign your time slot. Or, if you are running late for your appointment, please call to let us know 1-888-296-GPHA (4742).
